I don't think so. It's a game that there aren't items to be collected that open secret screens. I guess it works like the hole warps in Penguin Adventure
Exits in Knightmare only appear when you have reached the target level in a previous attempt. This means after a power cycle, you will always have to go through each level, as Exit boxes will simply not show.
On the second run, there should be two Exit boxes per level at best. Each jumps you to a different target level.
The intention behind this design is to save the player from a lot of pain replaying the entire game to reach the last level he died in when he got a game over.. think of it just like "Continue" but in a more interesting way
Exactly there are no exits from stage 5 onwards.
I don't remember very well but I think some exits allow you to skip one stage and some others two stages.
The last exit you can take is in stage 4 skipping to stage 6 (someone please correct me if I'm wrong).
So yes, you are forced to complete stages 6, 7 and 8.
After you finish loop 1, the game starts again with increased bullet speed.
Not sure if 3rd loop is the same or even more difficult than 2nd loop.
